WHY does this happen? Has anyone figured out exactly what causes this and how to stop it?

Try raising the entity a bit above the floor, see if that helps, if not make sure you didnt fuck up the floor somehow

Problem solved... in some rare cases it seems you actually have to move floors to entity func_wall... NO idea why this is or what causes it to happen.. But moving it to entity solved the problem for me.


(EDIT) No it did not solve the problem.... it did stop it falling through... but now it is considered a leak >.>.... I'll have to noob box that section I guess.

You don't need a noob box to seal it up, just make a solid brush that's just large enough to seal up the leak, making a "noob box" is probably one of the worse things you can do for the maps fps performance, and compile speed.
